The information below is required for all students enrolling at PVD Prep. Once you hit "send" below we will respond to your email with additional materials for you to complete.

Your child will not be fully enrolled the school has received ALL of the following (1) a copy of the child’s birth certificate (2) two separate documents that demonstrate your residence (3) updated immunization records (within the last 12 months) (4) RI School Physical Form (within last 12 months) (5) completed PVD Prep Health History (6) signed Medication Authorization (7) a completed application for Free or Reduced Price lunch, and (8) a copy of your child’s IEP or Section 504 plan, if applicable.

From time to time Providence Prep may employ licensed mental health providers who can offer both individual and group counseling. We may also use various screeners to identify potential social and emotional needs like managing stress and anxiety. By signing below, you give permission for the student applicant to participate in support services provided by Providence Prep.

PVD Prep has limited yellow bus service available to eligible students based on the distance of their home address from teh school. Please respond "yes" below if you require yellow bus service and give permission for your child to ride the yellow school bus to and from school if they are eligible.
